Health Exams

A health examination/physical examination including immunizations is required by state law. The law applies to students entering an early childhood or preschool program, students entering kindergarten, 6th and 9th grade. The examiner records the results of the health examination and immunizations on the Certificate of Child Health Examination. The Illinois Department of Human Services has revised the State of Illinois Certificate of Child Health Examination. This form is available under the 'Health Forms' tab of the district website. The health history section of the form shall be completed and signed by the student’s parent/legal guardian and verified by the physician. The completed form shall be presented to the school prior to the first day of attendance or the student shall be excluded from school.

Diabetes screening shall be included as a required part of each health examination and the health care provider shall document results of the diabetes risk assessment on the Certificate of Child Health Examination. Lead screening using the Lead Risk Questionnaire on the Certificate of Child Health Examination is a required part of the health examination for children age six years or younger prior to admission to a preschool, an early childhood, or a kindergarten program.

A tuberculosis (TB) skin test is recommended only for children in high-risk groups including children who are immunosupressed due to HIV infection or other conditions, recent immigrants from high prevalence countries, or those exposed to adults in high-risk categories according to CDC guidelines.
